Appointments may be scheduled by calling our centralized scheduling at 806-351-8484.
You will be directed to the appropriate location for your examination. For your convenience, we have bilingual staff.
You may download a patient information form to fill out in advance of your appointment and fax it to 806-351-8489 which will expedite your sign in process. If you prefer, you may bring the completed forms with you when you come in for your appointment that day.
